Commission assigning George Voorhis as surgeon's mate in the 122nd Regiment. Signed by Silas Wright and A.C. Niven. On reverse is statement signed by Major David G. Mallery of the 122nd Regiment, 11th Battalion, and 14th Division of Infantry to the effect that Voorhis appeared before him 1845 March 29 to take his oath.
